When we receive a request for treatment, the coordinator arranges the first appointment to identify the problem and makes the initial assessment. Depending on the problem, it may be necessary to complete this assessment by the team’s professionals. Once this first approach has been completed, the professionals who will take part in the specific treatment will meet, and the problems, objectives, and intervention plan will be agreed upon. We prepare an estimated quote and the proposal is written and presented to the patient and his/her family. At this meeting, the intervention and the date of the first review are agreed.
The evolution of each person is reviewed monthly with all the specialists involved in the treatment.
At the time to be discharged, we prepare a report, in which each professional who has taken part in the treatment provides the corresponding content. The coordinator delivers this written report to the patient in person.
